It is now time to fill out the BOTVIN forms for your schools. Below is a picture of that application, and what should be put into the appropriate boxes. If you are new to this process, Botvin LifeSkills Training is the only approved program that meets the requirements outlined in UCA § 53G-10-406 and R277-910. The program must be offered to all students in grades 4 or 5, 7 or 8, and 9 or 10 (UCA § 53G-10-406). In Salt Lake City School District, we begin our program in 5th grade.
All 5th grade teachers must receive the BOTVIN training. If a teacher is new to the district, or has been teaching in another grade, please make sure they get notified to take the training through Clarissa Stebbing. Required Teacher Training -New Simplified Registration Process
